INSTRUCTIONS FOR SUBMITTING AN RODP GRADE CHANGE

Follow your institution's policy for submitting a grade change form when acquiring signatures. The schedule for revising grades of Incomplete is included below. Please note that the deadline depends upon the student's home school rather than the delivery institution.

  1. Enter the instructor's first and last name, e-mail address, and phone number as well as the departmental dean's and chairperson's e-mail addresses and telephone numbers.

  2. Enter student ID, Last Name, and First Name, Middle Initial.

  3. Subject Area = ex. ENGL, BIOL, PSY, etc.

  4. Course # = ex. 2420, 3240, 1010, etc.

  5. Choose semester in which the original grade was assigned from the drop-down list.

  6. Choose year in which the original grade was assigned from the drop-down list.

  7. Select the original grade that was assigned from the drop-down list.

  8. Select the new grade to be assigned from the drop-down list.

  9. Select the course credit hours from the drop-down list.

  10. For changing an Incomplete grade to a regular grade, give the date the student completed the work.

  11. Print the form.

  12. Follow the grade change policies at the instructor's institution for required signatures.

  13. Return the form to your RODP Campus Contact for signature verification. Contact information for each Campus Contact may be found below.

  14. After verifying grade change policy at instructor's institution has been followed, the campus contact will sign and fax the form to the RODP Campus Contact at the student's home institution. Then follow-up with an email to the receiving campus contact telling them the form was sent and which fax number was used.

  15. The campus contact at the receiving institution verifies the form's legitimacy using contact information provided by sending institution, then he or she signs and sends it to his/her Registrar's Office.
